What does each claim status mean?

Every claim walks the same map, and the status is where it has got to. Learn these once and you can read any claim at a glance.

• Draft. You are still building it, and nothing has left your office.
• Sent, waiting on payer. It is out of your hands, sitting with the insurance company.
• Accepted by payer. They confirm it arrived and it is in their queue to be decided.
• Rejected by clearinghouse. It never got in at all, because something on the claim was wrong. This is not a denial. Fix and resend.
• Paid. The money arrived. Partially paid means only some of it did.
• Denied, needs your attention. They read the claim and said no. See A claim came back denied. What do I do?.
• Rejected by payer, needs your attention. The payer would not take it in.
• Payment reversed, needs your attention. Money already paid to you has been taken back.
• Written off. You have decided to stop chasing it, and the amount comes off your books.

Notice the pattern: any status ending in “needs your attention” is red, and it is asking you to do something. Everything else is just telling you where things stand.
