Where did this number come from?

Every number in the system came from somewhere, and you can always ask it where.

A claim knows how it was born. The Source column on the claims list answers it for every row: manual means a person keyed it in, bill import means it came in from a file, agent means the assistant created it, legacy means it predates our tracking. On the claim page itself, one plain line under the title says the same thing.

A claim knows its whole story. The Lifecycle section lists every step it took, oldest first, each stamped with the time it happened. This history only ever grows. A correction is a new line, never an edit. Nothing is deleted and nothing is rewritten.

A payment knows its source. A posted payment did not appear by magic. It says whether it came from an electronic remittance or was entered by hand, with a trace number and a date tying the money back to that document.

Coverage knows who entered it. Each insurance on a patient carries a stamp: entered by a teammate, on a date, and last checked by an eligibility check on another date.

If you are unsure, just ask. Ask where a payment on a claim came from and the assistant answers from that claim alone, dollar by dollar.
